FAMILY PEER SUPPORT SERVICES WORKER
Full Time/Part Time Regular Full Time Total Hours Per Week 35 From Salary $37,000 To Salary $37,000
Job Summary: The Family Peer Support Service Worker assists the program in providing services to families in the areas where support and advocacy is needed, but do not require the full services of the Care Manager.
Duties & Responsibilities
Comply with New York State office of mental health regulations. Assist in the intake and discharge processes of families. Conference on a regular basis with the Care Manager and supervisors. Home visitation. Accompany families to appointments to serve as a bridge between families and service providers. Supporting and assisting families during stages of transition i.e. placements, between service systems.
Education & Experience
Family Peer Support will be delivered by a New York State Credentialed Family Peer Advocate (FPA). To be eligible for the FPA Credential, the individual must:
Demonstrate ‘lived experience’ as a parent or primary caregiver who has navigated multiple child serving systems on behalf of their child(ren) with social, emotional, developmental, health and/or behavioral healthcare needs.
At a minimum, have a high school diploma, high school equivalency preferred or a State Education Commencement Credential. This educational requirement can be waived by the State if the person has demonstrated competencies and has relevant life experience sufficient for the peer credential.
Physical Requirements
While performing the duties for this job, the employee is regularly required to communicate clearly and effectively. The employee is required to stand; use mobility, handle, or feel and reach. The employee is occasionally required to sit and stoop, kneel or crouch. The employee must occasionally lift and/or move up to 25 pounds.

Salary : $37,000
