Position Summary:

  • Under the supervision of Public Health Director, responsible for writing the Special Diabetes Program for Indians grant with assistance from grant writing office. Perform all grant reporting. Assist the Public Health Nurse-Diabetes Health Grant with the coordination of services for diabetic clientele. Implement programs and projects to provide better quality services for clients. All Public Health staff must adhere to mandated reporting laws. This is a grant funded position.

Essential Duties and Responsibilities:

  • Provide Public Health Nursing diabetes self-management education services to eligible diabetic patients residing within the five county service area of the Saginaw Chippewa Indian Tribe.
  • Maintain a diabetes registry of all diabetic, pre-diabetic, and metabolic clients eligible for care and patient recall system for patients within the service area.
  • Maintain the required diabetes audit tracking system and submit information yearly. Establish and maintain the SDPI Outcome System (SOS) required by the SDPI grant for tracking of Best Practice Key Measures and submit twice a year.
  • Provide an ongoing update of the diabetic patient care plans to meet Indian Health Service (IHS) Standards of Care for diabetic patients PCC Diabetes Management System. Create and maintain GUI scheduling package for diabetes and JVN/foot care clinics.
  • Establish baseline statistics to determine the prevalence of diabetes within the service area and generate ongoing statistics to track disease indicators.
  • Assist with development of patient education protocols and educational strategies.
  • Create Standard Operating Procedures and Policies as needed for diabetes program. Provide assistance for other health staff with Standard Operating Procedures and Policies as needed.
  • Assist health staff with the planning and implementation of diabetic functions: i.e. diabetic clinics, diabetes group education, and other programs.
  • Serve as an active member of EHR, QA / QI team, IPC 5 team, team leader of Public Health QA/QI and team leader diabetes team meetings.
  • Provide home visits as necessary for patient assessment, monitoring and coordination of health care. Provide medical transports as necessary.
  • Maintain documentation of patient services, statistical reporting and quarterly written reports in accordance with Nimkee Memorial Wellness Center’s standards, provide quarterly narrative written reports due to the Public Health Director no later than the fifth day after the end of the quarter. Conduct web audit required by May 30th of each year to DM Area Coordinator.
  • Work with Grants, Contract Manager, and Fiscal Officer for finalization of grant processes.
  • Provide back-up for other Public Health nurses as needed. Assist public health programs with health events, i.e. Healthy Heart
  • Attend Nimkee Memorial Wellness Staff meetings and other meetings as directed by the Public Health Director and participate in various health promotion/disease prevention committees and meetings to enhance patient care a Nimkee Clinic.
  • Advocate for diabetic clients upon request to provide choices in health care providers and referrals to specialist as needed.
  • Maintain CEU’s to keep currant certifications and/ licensures. Attend all required webinars and conference calls of the SDPI.
  • Cover absence and provide backup, if necessary, of Public Health team members to ensure continuity of care.
  • Adhere to strict patient confidentiality and comply with department policies and federal regulations under the Privacy rules of the Health Insurance Portability and Accountability Act (HIPAA).
  • Provide excellent customer service for all internal and external customers of the operations at all times. Provide solutions for customer concerns and continually focus on customer service as our top priority.
  • Must present a positive, professional demeanor and image at all times.

  • Contact with Health Educator for the purpose of informational education regarding diabetes.
Minimum Qualifications:
  • Registered Nurse.
  • One year of public health or nursing experience.
  • Program planning and coordination of prevention program experience.
  • Must be able to effectively communicate with clients and all members of the health care team and have good computer skills.
  • Must be able to pass background check to meet the employment eligibility requirements as they pertain to the position.
Desired Qualifications:
  • Bachelor’s degree in Nursing.
License, Certification, or Special Requirements:
  • State of Michigan Registered Nurse License.
  • Ability to obtain HIPAA Certification, Blood Borne Pathogens Certification, BLS Certification and Safety Training within thirty days of hire.
  • Must have a valid State of Michigan Driver’s License with the proper designation required for the vehicle utilized to perform the job duties; must be able to obtain and maintain a Tribal Driver’s License and qualify for coverage by SCIT auto liability insurance carrier.
  • Native American preference shall apply to all positions.
Knowledge, Skills, and Abilities:
  • Knowledge of applicable federal, state, county and local laws, regulations, and requirements.
  • Knowledge and ability to apply principles, concepts and theories, core functions and essential services of Public Health Nursing to provide nursing care services in homes, schools, and community settings.
  • Knowledge of adult learning and health education techniques and principles.
  • Knowledge of diabetes nursing principles.
  • Knowledge of health care technology, equipment, supplies and materials needed for medical treatment.
  • Skill in treating patients with tact, courtesy, and respect.
  • Skill in operating business computers and office machines, including in a Windows environment, specifically Word, Excel, Access, PowerPoint, and EHR.
  • Ability to demonstrate professional conduct and upholds the nursing practice standards of care set forth by the State Board of Nursing.
  • Ability to obtain accurate vital signs.
  • Ability to react quickly and make effective decisions in an emergency situation.
  • Ability to interact and maintain good working relationships with individuals of varying social and cultural backgrounds.
  • Ability to communicate efficiently and effectively both verbally and in writing.
  • Ability to handle multiple tasks and meet deadlines.
  • Ability to maintain quality, safety, and/or infection control standards.
Physical Demands:
  • Frequent use of hands, wrists, fingers associated with providing medical care and with computer equipment.
