As a Qualified Health Home Specialist, you will serve as a coach whose main focus is to support and assist clients in reaching their goals. They provide ongoing motivation, encouragement, and positive feedback when client makes constructive changes or progress. With the guidance of their supervisor and in collaboration with the BHH team, the Qualified Health Home Specialist is the primary entity responsible for the Health Promotion Services and assists the BHH Systems Navigator in coordinating care, serving as the secondary entity responsible for care coordination Services.

- Responsible for ensuring clients are aware of resources and are supported in efforts to access resources in order to address the client’s identified goals and needs (i.e., county social services, housing, employment)
- Assist clients in identifying goals for functional improvement and work with them to meet their goals.
- Responsible for providing Health Promotion Services encouraging and supporting healthy living concepts to motivate clients and/or their caregivers to adopt healthy behaviors and promote better management of their health and wellness.
- Assist the BHH Systems Navigator in coordinating care, serving as the secondary entity responsible for care coordination services.
- In order to be considered for this position, you must be qualified in at least one of the following ways:
- Must pass DHS background study
- Be in possession of a valid driver’s license, auto insurance, and has reliable transportation
- Ability to drive a minimum of a 30-mile radius around assigned primary clinic location
- Must be proficient in the use of computers, specifically Microsoft Word, Excel, the use of email.
- Proficient with electronic health records
- Ability to meet with clients in their physical environment, this includes:
- Walking and climbing stairs
- Ability to crouch, stoop, and stand for periods of time
- A mental health certified peer specialist who is qualified according to Minnesota Statutes, 245I.04, subdivision 10
- A mental health certified family peer specialist who is qualified according to Minnesota Statutes, 245I.04, subdivision 12
- A case management associate as defined in Minnesota Statutes, 245.462, subdivision 4, paragraph (g) or Minnesota Statutes, 245.4871, subdivision 4, paragraph (j)
- A mental health rehabilitation worker who is qualified according to Minnesota Statutes, 245I.04, subdivision 14
- A community paramedic as defined in Minnesota Statutes, 144E.28, subdivision 9
- A peer recovery specialist as defined in Minnesota Statutes, 245G.07, subdivision 1, paragraph (a), clause (5)
- A community health worker as defined in Minnesota Statutes, 256B.0625, subdivision 49
If BHH services are offered in either a primary care setting or mental health setting, the qualified health home specialist must meet one of the following qualifications:
