position in Colebrook, NH. We are accepting both local and travel candidates.

Pay Rate: Range $2720 to $2800 weekly Gross Pay based on a 40-hour work week (Includes Non-taxable
travel/housing and M&I stipend available for qualified/eligible travelers)
Unit: Hospital based outpatient clinic
Shifts available: Days
Schedule: 40 hours/week guaranteed, 5 X 8-hours, Monday-Friday, No weekends, No call
Position Requirements:
- Physical Therapist experience: 2 years - Required
- PT Outpatient experience - Required
- PT Inpatient experience - Preferred
- Prior Travel experience – Preferred
Education, License, and Certification Requirements:
- PT License for the State applying or Compact if applicable - Required
- BLS Certification - Preferred
Job Description:
- Provides all
aspects of PT Services in a hospital based outpatient clinic and occasionally inpatient.
- The physical
therapist will accurately assess, differentially diagnosis, and identify
deficits in a patient’s ability to move and perform functional activities in
their daily lives.
- Develop individualized
treatment plans and performs evaluations.
- Establishes an
individualized plan of care and selects treatment techniques that addresses the
specific impairments identified in the evaluation and in accordance with
physician orders.
- Conducts
ongoing reassessment of the patient and their response to treatment.
- Completes
legible, accurate and clear documentation within the established timeframe
- Assesses
educational needs of the patient, family, caregiver, or significant other and
provides education to meet those needs.
- Functions
within scope of practice as defined by the state’s practice act.
- Performs other
related duties as required and directed.
