About the Role:

As a Physical Therapist in Home Health, you will play a critical role in delivering personalized rehabilitation services to patients within their own homes, promoting recovery and enhancing their quality of life. You will assess patients' physical conditions, develop tailored treatment plans, and implement therapeutic interventions to improve mobility, reduce pain, and prevent disability. This position requires collaboration with interdisciplinary healthcare teams to ensure comprehensive care and effective communication with patients and their families to educate and motivate them throughout the recovery process. You will be responsible for documenting patient progress and outcomes accurately to support ongoing care and compliance with regulatory standards. Ultimately, your work will empower patients to regain independence and achieve optimal functional outcomes in a comfortable and familiar environment.

Minimum Qualifications:

  • Valid and current state license to practice as a Physical Therapist.
  • Degree in Physical Therapy from an accredited program (Bachelor’s or Master’s degree as required by state).
  • Experience or clinical training in home health or outpatient physical therapy settings.
  • Strong knowledge of physical therapy principles, techniques, and rehabilitation protocols.
  • Ability to travel to patient homes within the designated service areas.

Preferred Qualifications:

  • Certification in Home Health Physical Therapy or related specialty.
  • Experience working with diverse patient populations including elderly and post-surgical patients.
  • Familiarity with electronic medical records (EMR) systems and documentation software.
  • Strong interpersonal and communication skills to effectively engage patients and multidisciplinary teams.
  • CPR and First Aid certification.

Responsibilities:

  • Conduct thorough patient evaluations to assess physical abilities, limitations, and rehabilitation needs in a home setting.
  • Develop and implement individualized treatment plans based on patient goals, medical history, and clinical best practices.
  • Provide hands-on therapy and instruct patients and caregivers on exercises and techniques to promote recovery and prevent injury.
  • Monitor patient progress regularly, adjusting treatment plans as necessary to optimize outcomes.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and ensure continuity.
  • Maintain detailed and accurate documentation of patient assessments, treatments, and progress notes in compliance with regulatory requirements.
  • Educate patients and families on health maintenance, injury prevention, and adaptive strategies to support long-term wellness.

The required skills enable the Physical Therapist to perform comprehensive patient assessments and develop effective treatment plans tailored to individual needs. Strong clinical knowledge and hands-on therapy skills are essential for delivering high-quality care and facilitating patient recovery in a home environment. Communication skills are critical for educating patients and collaborating with healthcare teams to ensure coordinated care. Proficiency with documentation and EMR systems supports accurate record-keeping and compliance with healthcare regulations. Preferred skills such as certification in home health therapy and experience with diverse populations enhance the therapist’s ability to address complex cases and improve patient outcomes.
