About the Role:

The Occupational Therapist-Home Health plays a critical role in delivering personalized therapeutic care to patients within their home environments, aiming to enhance their functional independence and quality of life. This position involves comprehensive assessment, treatment planning, and implementation of interventions tailored to each patient's unique needs, often addressing physical, cognitive, and psychosocial challenges. The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure continuity of care and to support patients in achieving their rehabilitation goals. By providing education and adaptive strategies, the therapist empowers patients to safely perform daily activities and participate fully in their communities. Minimum Qualifications:

• Valid state licensure as an Occupational Therapist.
• Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
• Current certification in Basic Life Support (BLS).
• Experience or clinical training in home health or outpatient occupational therapy settings.
• Strong knowledge of occupational therapy principles, techniques, and documentation standards.

Preferred Qualifications:

• Experience working in home health care or community-based rehabilitation.
• Certification in specialized areas such as hand therapy, neurological rehabilitation, or geriatrics.
• Familiarity with electronic medical records (EMR) systems and telehealth platforms.

Responsibilities:

• Conduct thorough evaluations of patients' physical, cognitive, and emotional abilities within their home environments to identify therapy needs.

• Provide hands-on therapy and training to patients and caregivers on adaptive techniques, assistive devices, and safety measures.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate care and optimize patient outcomes.
• Document patient progress accurately and timely in compliance with regulatory standards and organizational policies.
• Educate patients and families on health maintenance, injury prevention, and strategies to manage chronic conditions effectively.
