Position Overview
Responsibilities:
- Care Oversight: Monitor and oversee the care provided by field staff to ensure compliance with care plans and safety protocols.
- Coordination: Collaborate closely with physicians, clinicians, and other healthcare professionals to develop and implement patient care plans.
- Patient Advocacy: Serve as a liaison between patients, families, and healthcare providers to advocate for the patient’s needs and preferences.
- Documentation: Maintain accurate and timely records of patient care, including assessments, care plans, and follow-up documentation.
- Compliance: Ensure all care provided meets state and federal regulations as well as company policies.
- Experience: Minimum of 2 years of experience as a Home Health RN.
- EMR Proficiency: Experience using WellSky (Kinnser) EMR for home health is preferred.
- Licensure: Current RN license in the state of Illinois.
- Skills: Strong organizational, communication, and problem-solving skills. Ability to work independently while managing multiple priorities.
