Key Responsibilities
- Clinical Care Management: Conduct comprehensive patient assessments, develop individualized care plans, and coordinate multidisciplinary healthcare services

Licensure & Certifications:
- Valid Illinois Registered Nurse license
- Current CPR certification
- Valid driver's license and auto insurance
