Responsibilities
- Process new patient referrals and request / follow up on required clinical information/document
- Communicate with referral source, healthcare providers, patients, and families
- Schedule appropriate care staff based on patient needs and orders
- Ensure compliance with healthcare regulations and company policies
- Track referral sources and assist with relationship management
- High school diploma required; Associate's degree in healthcare or related field preferred
- Experience in healthcare administration, especially in home health settings
- Knowledge of medical terminology and insurance verification processes
- Excellent organizational and time management skills
- Strong communication and interpersonal abilities
- Proficiency with electronic health records and office software - training in working with Kinnser EMR is preferred.
