Responsibilities
- Provide strong clinical skills, excellent communication abilities, and a passion for patient-centered care
- Understands and adheres to established policies and procedures
- Conduct comprehensive assessments of patients' health status and needs in their home environment
- Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition
- Develop and implement individualized care plans based on patient assessments and medical history
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols
- Monitor patients’ progress, documenting changes in condition and response to treatment
- Provide education to patients and families/significant others regarding disease process and management, medication regimen, pain management, fall prevention, infection control, proper diet / exercise, when to contact pharmacy / physician / SN and when to seek emergency care.
- Collaborate with interdisciplinary teams to coordinate care, including discharge planning and referrals to other services as needed
- Maintain accurate medical records and documentation in compliance with legal and regulatory standards
- Respond promptly to patient inquiries or concerns, providing support and guidance as necessary
- Attends staff meetings, patient care conferences and in-services as scheduled
- Valid Illinois RN license
- Valid CPR Certification
- Valid Drivers License
- Valid Auto Insurance
- Minimum 1 year home health experience
- Proficiency in WellSky Kinnser EMR
