Safe Haven Family Resource Center, Inc. is seeking a compassionate, organized, and dependable Part-Time Foster Care Case Manager to provide individualized placement monitoring and support to children and youth residing in Safe Haven foster homes.
The Case Manager’s primary responsibility is to conduct face-to-face home visits and ongoing placement-monitoring contacts based on each child’s individualized needs and assigned level under the Safe Haven Needs-Based Home Visit Tier System.
This position is designed to ensure that children receive the appropriate level of monitoring and that foster parents receive timely support based on the child’s behavioral, emotional, medical, educational, safety, and placement-stability needs.
The Case Manager will work collaboratively with foster parents, children and youth, Safe Haven leadership, DCBS workers, schools, behavioral-health providers, and other members of the child’s treatment team to promote safe and stable foster-care placements.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Home Visits & Placement Monitoring
- Conduct scheduled in-person visits with children and foster families according to each child’s assigned Safe Haven Needs-Based Home Visit Tier.
- Complete additional visits when a child’s behaviors, safety needs, placement stability, or caregiver support needs increase.
- Maintain required contact frequency for assigned Tier 1, Tier 2, Tier 3, and Tier 4 placements.
- Meet individually with children during home visits when developmentally appropriate.
- Assess the child’s safety, well-being, adjustment, and overall experience within the foster home.
- Observe the home environment and identify changes that may affect the child’s safety or placement.
- Identify concerns early and communicate them to appropriate Safe Haven leadership.
REQUIRED QUALIFICATIONS
- Bachelor’s degree in social work, psychology, human services, sociology, family studies, criminal justice, or a related behavioral/social-service field, or other education and experience meeting applicable agency and regulatory requirements.
- Experience working with children, adolescents, families, foster care, behavioral health, child welfare, or other human-service populations preferred.
- Ability to work independently in foster homes and community settings.
- Strong assessment, observation, communication, and documentation skills.
- Ability to establish professional relationships with children, foster parents, and multidisciplinary professionals.
- Ability to respond calmly and professionally to challenging behaviors and placement concerns.
- Understanding of trauma-informed care and the impact of trauma on children and families.
- Ability to maintain professional boundaries and confidentiality.
- Ability to complete documentation within established deadlines.
ADDITIONAL REQUIREMENTS
The employee must:
- Successfully complete all required background and child-abuse/neglect checks;
- Maintain a valid driver’s license and reliable transportation;
- Maintain automobile insurance as required by agency policy;
- Be willing to travel to foster homes, agency offices, and community locations;
- Complete required Safe Haven orientation and training;
- Maintain required annual training hours;
- Participate in supervision and case consultation;
- Maintain CPR/First Aid certification when required;
- Be available for some evening and weekend home visits based on foster-family schedules; and
- Comply with Safe Haven policies, Kentucky requirements applicable to the position, confidentiality requirements, and mandatory-reporting laws.

WORK SCHEDULE
This is a part-time position with scheduling based primarily on the needs and availability of assigned children and foster families.
Because many foster children attend school during traditional business hours and foster parents may work during the day, the position requires evening availability and occasional weekend availability.
The number and frequency of home visits may fluctuate as children move between monitoring tiers.
PERFORMANCE EXPECTATIONS
The Part-Time Foster Care Case Manager is expected to:
- Complete assigned home visits within required timeframes;
- Maintain required contact frequency based on each child’s assigned tier;
- Submit timely and accurate documentation;
- Escalate safety and placement-stability concerns promptly;
- Maintain professional communication with children, foster parents, and team members;
- Follow through on identified needs and assigned action items; and
- Support Safe Haven’s goal of maintaining safe, stable, individualized foster-care placements.
PHYSICAL & FIELD REQUIREMENTS
This position requires regular travel within the service area and entry into foster homes and community settings. The employee must be able to perform routine office and community-based duties associated with case management and home visitation.
Pay: $32,000.00 - $39,104.00 per year
