Hospice Care Team Member

Are you a compassionate individual who finds purpose in supporting others during life's most meaningful moments? Faith Home Health & Hospice is seeking a caring, dependable Care Team Member to provide end-of-life vigil support to hospice patients and their families.

As a Care Team Member, you will offer comfort, companionship, and emotional support while working alongside an interdisciplinary hospice team dedicated to providing exceptional care and dignity at the end of life.

The Care Team Member provides non-medical companion care and vigil support to hospice patients. This role focuses on ensuring patients are not alone during critical times and providing a calming, supportive presence for patients and their loved ones.

• Provide endoflife vigil and companion support services to hospice patients.
• Observe and communicate patient status updates to the hospice team.
• Collaborate with nurses and other care team members to ensure quality patient care.
• Follow all hospice policies, procedures, and infection control protocols.
• Attend care team meetings as requested.
• Complete required orientation and hospice training.
• Maintain professional, compassionate interactions with patients and families.
• Perform other duties as assigned by the Director.
QUALIFICATIONSRequired:
• High school diploma or equivalent
• Ability to work independently and exercise sounds judgment
• Strong interpersonal and communication skills
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to adapt to diverse social, cultural, and religious backgrounds
• Ability to respond appropriately in emotionally sensitive situations
Preferred:
• At least one year of experience in healthcare, caregiving, companion care, or a related field.
• Previous hospice, palliative care, companionship, or volunteer experience
PHYSICAL REQUIREMENTS
• Frequent walking, standing, bending, reaching, kneeling, and squatting.
• Ability to assist with lifting or transferring up to 20 pounds.
• Ability to work in home environments with varying conditions and exposure to common healthcarerelated odors and materials.
