About The Role

  • Sabathani Community Center’s Health Equity Department is implementing the Quit Partner™ Ambassador program in partnership with the Minnesota Department of Health. The program promotes commercial tobacco cessation resources and supports community members in enrolling in Minnesota’s Quit Partner™ cessation services through culturally responsive outreach and education across South Minneapolis.
  • Sabathani Community Center is seeking a Quit Partner™ Coordinator to lead the implementation of its Quit Partner™ Ambassador program. The Coordinator will recruit, train, and support trusted community members to serve as Quit Partner™ Ambassadors, coordinate outreach and education activities across South Minneapolis, and support community members in accessing tobacco cessation services. This position reports to the Director of Health Equity and is funded through a grant from the Minnesota Department of Health.

What You'll Do

Key Responsibilities:

  • Ambassador Recruitment & Training
    • Recruit trusted community members from faith institutions, barbershops, community organizations, and Sabathani programs to serve as Quit Partner™ Ambassadors.
    • Develop and facilitate ambassador training using MDH-approved materials and culturally responsive messaging appropriate for the communities served.
    • Provide ongoing coaching, support, and supervision to ambassadors throughout the grant period.
    • Maintain records of ambassador recruitment, training completion, and outreach activities.
  • Community Outreach & Education
    • Plan and coordinate outreach activities at community sites including faith institutions, barbershops, service sites, and Sabathani programs.
    • Conduct one-on-one conversations, tabling, and small group education sessions to increase community awareness of Quit Partner™ services, eligibility, and enrollment.
    • Support community members in enrolling in Quit Partner™ services on-site or through follow-up assistance, and maintain a referral tracking system to monitor reach and effectiveness.
  • Partner Coordination & Administration
    • Establish and maintain relationships with community partner sites and facilitate quarterly check-ins to review outreach data, gather feedback, and adjust strategies as needed.
    • Integrate Quit Partner™ promotion into existing Sabathani programs including food distribution, maternal health, senior services, workforce development, and parenting education.
    • Track and document outreach activities, ambassador hours, referrals, and community engagement data for grant reporting purposes.
    • Provide regular updates and reports to the Director of Health Equity on program progress, challenges, and outcomes.
    • Attend MDH Quit Partner™ training and technical assistance events as required and perform other duties as assigned.
  • Scheduling & Flexibility
    • Be available for flexible scheduling, including evenings and weekends, to meet the community where it is.
    • Coordinate outreach schedules around existing community gatherings, service days, and high-traffic periods to maximize reach.
Qualifications

  • Education:
    • Bachelor’s degree in community health, public health, human services, social work, or a related field; or equivalent lived and professional experience.
  • Experience:
    • Minimum two years of experience in community health education, health promotion, outreach, or a related field.
    • Experience recruiting, training, and/or coordinating community volunteers or peer ambassadors.
    • Knowledge of commercial tobacco cessation resources, including Minnesota’s Quit Partner™ program, preferred.
    • Strong facilitation, organizational, and written communication skills.
    • Familiarity with the social determinants of health and structural factors influencing health outcomes in underserved communities.
    • Must pass a criminal background check.
    • Flexibility to work evenings and weekends as needed.
  • Knowledge, Skills and Abilities:
    • Ability to communicate clearly and effectively via oral or written means.
    • Ability to remain calm and maintain self-control in the midst of difficult circumstances and emergencies.
    • Ability to adapt to change. Ability to respond in a professional manner in all situations. Working knowledge of Outlook, Word and Excel; PowerPoint and Access a plus.
    • Ability to think analytically and translate community feedback into actionable program strategies.
    • Ability to set work priorities and to evaluate and create solutions to work related problems.
    • Ability to build trust and maintain professional relationships with community members and partner organizations.
    • Knowledge of health promotion strategies, community outreach models, and tobacco cessation resources.
  • Licenses and/or Certifications:
    • No specific licensure required. Completion of MDH Quit Partner™ ambassador training required upon hire.
    • Valid MN Driver’s License, access to reliable transportation, and ability to meet Sabathani’s driving record requirements preferred.
  • Working conditions and Environment:
    • This position is based at Sabathani Community Center with significant work conducted in community settings across South Minneapolis, including faith institutions, barbershops, and community events. The building is accessible and smoke-free. Scheduling is flexible and coordinated with program needs, with some evening and weekend hours required. This position is grant-funded through the Minnesota Department of Health Quit Partner™ Ambassador program.
