Position Summary
We are seeking a compassionate, clinical expert to join our team as a Weekend Admissions Nurse. In this role, you are the first face of hospice for patients and families during some of their most difficult moments. You will be responsible for conducting initial assessments, explaining hospice benefits, and ensuring a seamless transition into our care during the high-demand weekend hours.
This "Weekend Warrior" schedule offers the perfect balance for someone looking to maximize their impact over three days while enjoying four days off during the week.
Key Responsibilities
Initial Consultations: Meet with patients and families in private homes, hospitals, or skilled nursing facilities to explain the hospice philosophy and services.
Clinical Assessment: Perform comprehensive physical and psychosocial assessments to determine hospice eligibility.
Documentation: Complete all admission paperwork and electronic health records (EHR) accurately and timely, ensuring compliance with Medicare/Medicaid regulations.
Collaboration: Work closely with the On-Call Triage team and Hospice Team to initiate the Plan of Care and order necessary medications/equipment.
Emotional Support: Provide immediate crisis intervention and education to ease the anxiety of transitioning to end-of-life care.
Qualifications
Licensure: Active Registered Nurse (RN) license
Experience: Minimum of 1–2 years of experience in hospice, home health, oncology, or ICU/ER settings preferred.
Skills: Exceptional communication skills and the ability to discuss end-of-life care with sensitivity and clarity.
Autonomy: A self-starter who thrives in an independent, mobile work environment.
Requirements: Reliable transportation, valid driver’s license, and automotive insurance.
