Here's what you'll be doing
You'll be responsible for helping patients overcome psychological barriers to their cardiometabolic health, including:
- Assessing social determinants of health needs and developing a treatment plan
- Providing personalized coaching, guidance, and encouragement to help individuals set achievable goals and develop healthier habits
- Collaborating with a multidisciplinary team to create tailored strategies that promote resilience, self-awareness, and overall wellness
- Creating and maintaining documentation
- Building rapport and maintaining positive relationships with patients and their families
- Over 2 years of experience as health or life skills coach with a strong focus on overall mental health and well-being
- Excellent communication skills, empathy, and the ability to inspire positive change through motivational interviewing and other tools
- Demonstrated ability to partner with other clinicians in a multi-disciplinary care environment
- Passion for the impact of your work on the broader world, and excitement to help reduce the burden of chronic disease
- Master's-level coursework and/or experience supporting patients navigating chronic physical disease preferred but not required
