RVNAhealth is seeking a Full Time Hospice Licensed Practical Nurse (LPN) to join our dedicated hospice team. In this field-based role, you'll provide direct patient care under the supervision of a Registered Nurse, delivering both intermittent and continuous care visits in accordance with personalized care plans. You'll be part of a collaborative, interdisciplinary team that supports patients and families with compassion, dignity, and professionalism.
This is a full-time position (Monday-Friday, 8:30am - 4:30pm) and will cover the area of Southern Fairfield County, including Stamford and Greenwich.

Our target pay range for this position is $33.00 to $37.00 per hour represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
It will be a condition of employment at RVNAhealth to submit to a pre-employment physical/drug screen and background check, which may include the following: fingerprinting, verification of employment, paraprofessional certifications, designations or licenses, and educational background.

Key Responsibilities
- Deliver direct nursing care in accordance with physician orders and under RN supervision.
- Monitor and report patient status and changes to RN Case Manager and clinical leadership.
- Assist with updates to the nursing care plan based on patient needs.
- Participate in routine Interdisciplinary Group (IDG) meetings to ensure coordinated care.
- Provide emotional support and education to patients and families.
- Participate in the hospice's continuous care rotation as scheduled.
- Maintain compliance with Medicare, Medicaid (Title 19), and insurance documentation requirements.
- Use nursing equipment appropriately and maintain accurate electronic documentation.
- Support and back up other hospice team roles when needed.
- Engage in ongoing quality improvement initiatives and in-service training.
- Graduate of an accredited nursing program.
- Active LPN license in the State of Connecticut.
- Minimum 2 years of clinical experience (MA, CNA, or similar role).
- Preferred: At least 1 year of hospice or home care experience.
- Strong understanding of community-based care and hospice regulations.
- Ability to manage multiple tasks and maintain professionalism under pressure.
- Comfortable operating a motor vehicle in all weather conditions.
- Computer-literate with proficiency in electronic documentation systems.
- This is a field-based role providing care in patient homes.
- Must be able to transport and use nursing equipment in varying home environments.
- Requires a valid driver's license and reliable transportation and the ability to drive in diverse weather conditions.
