Pay Range: $21.61 - $34.04 per hour

Summary:
The Care Management Navigator II is responsible for providing bilingual navigation services to patients engaged in RUSH MD Anderson Cancer Center’s breast surgery clinics. The Care Management Navigator will utilize the established model of care management to support patients engaged with these clinics by assessing and addressing patient barriers to care. The Care Management Navigator will provide knowledge, education, and care coordination to empower patients and improve their ability to navigate services and systems. This role will involve close collaboration with the breast surgery nurse navigator and other healthcare team members to ensure optimal patient care.
This position is part of the Supportive Oncology team, a multidisciplinary team supporting the Rush Cancer Center. - Provide bilingual navigation services to patients, families, and caregivers engaged with breast surgery clinics.
- Work directly with patients and their families to address acute barriers to care.
- Monitor new referrals and existing caseload of patients on a daily basis.
- Provide in-person and telephonic outreach as necessary to ensure care is delivered on a timely basis.
- Assess patients’ transportation and care coordination needs, along with any other barriers impacting their ability to attend appts.
- Develop action plans and basic care planning for low-risk patients.
- Implement interventions to ensure patients have support and resources to support successful appointments.
- Educate patients on available resources and be knowledgeable of available programs to address SDOH and other barriers to care.
- Provide assistance with insurance as needed, including connecting patients with resources for enrollment or lapses and problem-solving insurance challenges.
- Improve and further develop existing workflows.
- Ensure compliance with quality measures to improve identified outcomes (i.e. Health Risk Assessment, high utilizer patients, etc.).
- Collaborate with Supportive Oncology team, breast surgery staff, and other providers as needed regarding patient care needs.
- Acquire comprehensive knowledge of Supportive Oncology navigation services in order to provide navigation coverage and assist with team caseloads as needed.
- Engage in American Cancer Society’s Leadership in Oncology Navigation trainings and other professional development opportunities as offered to strengthen oncology navigation skills and knowledge.
- Document all patient interactions in Epic electronic health records system.
- Promote the Rush Supportive Oncology program and refer patients to internal Rush services as appropriate.
