We're pleased to announce an opportunity for the position of TXIX Case Manager**,** within the Senior and Independent Living Services Department.
**Position Summary**
Provides federally regulated Medicaid case management services under Title XIX programs in compliance with federal Medicaid rules, Washington Administrative Code (WAC), and agency policy. Responsible for conducting comprehensive CARE assessments, authorizing long-term care services, issuing Planned Action Notices (PANs), and ensuring regulatory compliance. Responsible for assessment of client needs, development and monitoring of service plans, makes appropriate referrals and serves as a customer advocate with other service providers. Assists with department day-to-day operations as delegated, including reports, outreach, community events, support groups, and more. Requires knowledge of related community resources and the laws and regulations specific to the area of service.

- Complete the initial interviews with clients and their families to assess the customer's eligibility for services.
- Gather relevant and useful data from the client, family, other agencies, to formulate a psychosocial assessment of the client and his or her family.
- Assemble and guide group discussions and decision-making sessions among relevant professionals and program representatives, clients and their families, and significant others to formulate goals and design an integrated intervention plan.
- Act as a liaison between the client and his or her family and all relevant professionals, programs, and informal resources involved in the overall intervention plan to help the client make his or her preferences known and secure the services needed.
