Works closely with the Program Development Manager to ensure project success and effective communication amongst all necessary program components. Maintains a timely and accurate record of responsibilities and completed tasks. Conducts housing supportive work with clients and helps to address social determinants and unmet social needs.

- Offered Wage Range:** $21.65-$23.42 per/hr.
- Full Wage Range:** $21.65 - $57.73 per/hr. (Represents the low and high end of the total salary range for this position)

- Conduct in-person visits with clients across designated service areas, requiring frequent travel by vehicle. This includes driving to client homes, community settings, and partner agencies to perform assessments, deliver case management services, and facilitate referrals.
