Job description:
This is an office-based position. Office is located in Queens.
JOB DUTIES /KNOWLEDGE
- Arrange/organize schedules for clients and patients in line with healthcare plans, needs of the patient, and availability of the healthcare professional that will care for the patient.
- Create daily schedules for home care nurses & therapist
- File patient information, answer telephone calls
- Maintain and update all computerized appointment schedules on a regular basis
- Address all client questions and issues completely and efficiently
- Run reports to monitor field staff productivity, missed visits, schedule, etc.
- Data enters clinical notes into EMR for visits performed by clinical vendors
- Train and Manage scheduling coordinators
- Tracks submission of all paper documentation from field staff and informs Clinical Manager if not received within prescribed time frame
- Scans incoming paper documentation into the patients record.
- Communicates effectively to other professionals within the agency and the patient’s physician and vendors
- Treats patients with dignity and respect and maintains a non-discrimination policy
- Collaborates with clinical and business staff, other agency departments and outside agencies to assist in achieving agency goals
- Assists in the performance of aide Exception Audits and resolution of Conflict Reports as directed by the Administrator.
- Adheres to the agency’s Code of Conduct
- Performs other related duties as assigned.

PERSONAL/PROFESSIONAL DEVELOPMENT
- Maintains current personnel and medical file information and provides information to agency in timely manner.
- Sets own development challenges and volunteers to learn.
- Assists with orientation of new personnel.
- Adheres to agency infection control and safety policies, including reporting, practice and implementation specific to OSHA mandates.
- Attends in-service programs to fulfill requirements of position and agency policies.
- Maintains stable performance and emotions when faced with opposition, pressure and/or stressful conditions.
- Understands role in the execution of the Emergency / Disaster Plan.
QUALIFICATIONS
- High School graduate
- Home care experience preferred
- Supervising experience preferred
- Proficient in Microsoft Word and Excel
- Excellent verbal and written communication skills
