Targeted Case Manager

POSITION: Targeted Case Manager

LOCATION: Elizabethtown, KY

QUALIFICATIONS:

• Bachelor’s degree in human services (or a related field)
• Targeted case management (TCM) certification
• Completion of case management training approved by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (KBHDID) within six months of employment
• Completion of recertification requirements (KBHDID) every three years
• Experience working as a TCM preferred

RESPONSIBILITIES AND TRAINING:

• Coordinate services for patients with severe emotional disabilities, mental illnesses, and substance use disorders
• Perform patient assessments to determine strengths and needs (medical, social, functional, educational)
• Perform periodic reassessments
• Arrange for transportation and delivery of services
• Facilitate referrals
• regularly communicate with patients about their health, well-being, and appointments
• Provide documentation (care plans, forms, reports, narratives)
• communicate with the Rosenwald outpatient treatment team
• Participate in weekly treatment team meetings
• Other duties as assigned
• Be prepared for crisis intervention, first aid, CPR (training provided)

SHIFT: Monday-Friday 8am-5pm est.
