LPN MDS Coordinator Urgent

Location: Williamsport, Pennsylvania

Rose View Nursing and Rehabilitation Center is seeking a detail-oriented and experienced LPN MDS Coordinator to join our clinical team on a part-time basis. As a key member of our interdisciplinary team, you will be responsible for assisting with the Resident Assessment Instrument (RAI) process, ensuring timely and accurate completion of Minimum Data Set (MDS) assessments, and supporting the development and maintenance of resident care plans.

LPN MDS Coordinator Key Responsibilities:

• Complete and coordinate MDS assessments in accordance with RAI guidelines and regulatory timelines

• Review resident medical records to ensure documentation supports assessment accuracy

• Collaborate with interdisciplinary team members to develop and update resident care plans

• Monitor and maintain the MDS schedule to ensure timely submissions

• Assist with Medicare and Medicaid case mix documentation and reimbursement processes

• Ensure compliance with CMS regulations and state requirements

• Communicate effectively with nursing leadership regarding assessment findings and resident status changes

• Support facility survey readiness and quality improvement initiatives

LPN MDS Coordinator Qualifications:

• Active Licensed Practical Nurse (LPN) license in Pennsylvania

• Previous MDS experience in a skilled nursing facility preferred

• Knowledge of RAI guidelines, care planning, and case mix reimbursement

• Strong attention to detail and organizational skills

• Ability to work collaboratively with interdisciplinary teams

• Proficiency with electronic medical record systems (EMR)
