- Monitor patient status and respond to clinical changes in collaboration with nephrologists.
- Partner with an interdisciplinary team to support comprehensive renal care.
- Educate patients and families on treatment plans, dialysis, and self-management.
- Support safe transitions of care across settings.
- Champion quality improvement and patient safety standards.
- Registered nursing licensure and strong clinical expertise in renal care and dialysis.
- Ability to coordinate complex care plans and lead within a multidisciplinary team.
- Strong patient communication and education skills.
- Sound clinical judgment, patient monitoring, and commitment to quality and safety.
