What You’ll Do:

• Deliver skilled nursing care and oversee pain and symptom management for hospice patients.

• Collaborate with physicians, chaplains, social workers, and other team members to develop and implement care plans.

• Educate patients and their families on the disease process, hospice philosophy, and available services.

• Provide emotional support and guide patients and families through the end-of-life journey.

• Maintain timely and accurate documentation in compliance with agency, state, and federal regulations.

• Participate in case conferences and on-call duties as needed.

What We’re Looking For:

• Active RN license in the state of Michigan.

• Minimum of one (1) year of nursing experience, preferably in hospice or palliative care.

• Compassionate communicator with strong interpersonal skills.

• Ability to travel within assigned coverage areas; reliable transportation, a valid driver’s license, and auto insurance are required.
