Home Care Coordinator
Schedule: Full Time Monday-Friday 8am-5pm
Pay: $22.00 - $29.57 / hour DOE
You will work in collaboration with the Home Care team to provide individualized care and coordinate services for clients of the Home Care program. You will work to ensure that resident needs are being met in a timely and efficient manner while maintaining and enhancing the quality of documentation by staff.

You will work in collaboration with the Home Care team to provide individualized care and coordinate services for clients of the Home Care program. You will work to ensure that resident needs are being met in a timely and efficient manner while maintaining and enhancing the quality of documentation by staff.
How You’ll Do It
- Develop and coordinate staffing schedules for all Home Care employees. Keep accurate documentation on staffing rosters, actual schedule of illnesses, no call/no shows, request for time off and absences.
- Complete status reports and communicate all personnel information on status changes, new hires, transfers, and terminations to HR Director.
- Notify managers regarding employees when necessary regarding missed punches and not taking breaks or lunches. Communicate staffing changes to Director of Health Services or alternates.
- Support hiring practices for Home Care by coordinating with HR on open positions and compensation, interviewing candidates and assisting with orientation for new employees.
- Manage and maintain central supply. Keep the central supply organized and clean, coordinate orders and stock supplies for Health Services.
- Check resident rooms to ensure DME, Oxygen, ancillaries, etc have been accounted for
- Answer telephones and direct calls for residents and staff. Coordinate client care schedules, appointments and licensed nurse visits for clients and residents.
- Track the service plans for residents and clients, making. the appropriate changes and recommendations. Maintain clients’ charts on a daily basis.
- Provide on-call support as needed, and consult with the Director to address emergencies and resident/staff issues.
- Assist in the evaluation of quality assurance programs. Audit the completion of the quarterly physician’s orders for clients receiving medication services.
- Assist in payroll, billing processes and administration for the team as needed. Assist with the development and implementation of protocols and systems for the program.
- Assist in the Intake Process of the resident including client evaluations, obtaining a signed disclosure statement, and developing client service plans.
- Review the communication books and gather information from the team, keeping care team apprised of changes in resident conditions, incident reports and other applicable patient information.
- Assist with scheduling of staff, new hire orientation and team trainings to ensure quality care delivery.
- Communicate with families and health care professionals regarding resident/client’s health to ensure coordination of care. Document communications as appropriate.
- Develop and implement necessary forms, privacy policies and procedures regarding the maintenance and destruction of records, and for responding to all complaints regarding noncompliance. Manage record keeping for the home care program.
- Work the floor as a caregiver and/or medication aide as needed
- Minimum of 1 year’s experience in home care, home health or the nursing services field.
- Must be CPR certified within the first year of employment.
- Strong computer skills, including managing medical records systems, scheduling software and Microsoft Office suite. Additional attributes to be a top performer:
