Position Purpose
The Outreach Specialist is responsible for conducting street and agency outreach for individuals with Intellectual and Developmental Disabilities (I/DD) who are experiencing homelessness in Denver. This position is part of the INCLUDE program (Intellectual and Developmental Disability Community Living for the Unhoused in Denver) to ensure an accountable and responsive service delivery model for people supported by RMHS. This position will complete all Intake and Eligibility documents with all clients. This staff member will be the INCLUDE program’s expert on local homeless outreach and engagement. This position will require daily travel throughout Denver.
Essential Duties
- Work closely with local homeless shelter providers to quickly identify individuals receiving shelter and connect them to INCLUDE.
- Provides essential customer service to potential applicants (those with I/DD) and community partners inquiring about the INCLUDE Program and answering questions regarding intake process, eligibility and types of services provided.
- Maintain/track application/screening data inflow and report outcomes.
- Conduct regular “street” outreach to form relationships with individuals experiencing homeless and connect them to services that end their homelessness.
- Provide resources for individuals like food and shelter options.
- Maintain a strong relationship with the City/County homeless outreach teams, other outreach programs, and regularly collaborate in community outreach efforts.
- Attend various local treatment courts when possible, and collaborate with other homelessness organizations locally in Denver.
- Maintain accurate, detailed documentation of client case files. Input required data into agency-based data system, and the Homeless Management Information System (HMIS).
- Complete eligibility documentation with individuals to get them enrolled into INCLUDE on a daily/weekly basis.
- Work closely with the Program Manager to assist individuals with the program eligibility determination process and refer individuals to other community services to meet their needs.
- Works with the Program Manager and other assigned staff to provide periodic audits of case files, HMIS entry data, and assisting in preparation for audits.
- Supports efforts of community coordinated entry to advocate for housing opportunities.
- Provide office coverage as needed to help support walk-ins.
- Ensure client confidentiality is maintained in accordance with established procedures and regulations.
- Maintains prompt and regular attendance.
- Perform other duties as assigned.
Knowledge, Skills, and Abilities
- Ability to evaluate individual’s needs, and display sound judgment in the conduct of duties within established policies and procedures.
- Ability to assess whether an individual has an I/DD (with training and support of the Program Manager)
- Ability to organize, train and lead volunteers on homeless outreach.
- Ability to work well with all levels of the organization.
- Ability to be self-directed and multi-task oriented.
- Ability to communicate effectively both orally and in writing.
- Ability to work independently with minimum supervision.
- Knowledgeable of Microsoft Office Suite
Qualifications:
Minimum Qualifications
- Bachelor’s degree in social work, social sciences, human services, or related field
- Ability to work independently while meeting work deadlines and program standards
- Experience working with diverse and vulnerable populations.
- Prior case management and/or homeless outreach experience
- Prior experience working with homeless individuals and/or households
- Valid driver’s license and proof of insurance.
Driving Category - Requirements
Driving Category B:
Employees who may drive RMHS or personal vehicles for business purposes. Category B employees may transport other employees but will not transport RMHS clients.
- Valid driver’s license
- Proof of motor vehicle insurance
- Personal vehicle in good operating condition for use during work, including transporting individuals
- No major violations in the past three years.
- No more than two moving violations in the past three years
- Ability to meet and maintain agency driving requirements and operate agency vehicles
- Drivers must upload proof of ongoing auto insurance every 6 months into Paycom/Relias.
Preferred Qualifications
- Master’s degree in social work, social sciences, human services or related field
- Experience working with I/DD populations
