Rock Valley Community Programs provides transitional and supportive housing, addiction and mental health treatment services to adults in the Rock County area. We provide these services to homeless male veterans, clients connected to the criminal justice system, and outpatient care to anyone needing assistance with addiction and/or mental health treatment.
Position:
The Residential SUD/DD Case Manager provides comprehensive case management, treatment coordination, and evidence-based group facilitation services to clients participating in a residential Substance Use Disorder (SUD) and Dual Diagnosis treatment program. This position supports clients who are working to address substance use disorders, mental health conditions, criminogenic risk factors, and barriers impacting successful community reintegration.
The Case Manager is responsible for managing an assigned caseload of approximately 8–12 clients while facilitating approximately 10–12 hours of structured group treatment programming per week. This position requires the ability to build therapeutic relationships, apply evidence-based interventions, reinforce pro-social behaviors, challenge maladaptive thinking patterns, and support clients in developing skills necessary for long-term recovery and reduced recidivism.
Schedule: Monday–Friday
Key Responsibilities:
  • Manage an assigned caseload of approximately 8–12 residential clients.
  • Complete regular individual sessions focused on:
    • Criminogenic needs
    • Behavioral patterns
    • Personal barriers to success
    • Reentry planning
  • Assist clients in identifying goals related to recovery, stability, employment, housing, relationships, and community reintegration.
  • Coordinate with Therapists, Program Leadership, medical staff, DOC personnel, and community providers to support continuity of care.
  • Monitor client progress and identify barriers requiring intervention.
  • Facilitate approximately 10–12 hours of group treatment programming weekly. Deliver evidence-based curricula designed to address substance use disorders, criminogenic needs, problem-solving skills, and emotional regulation.
  • Facilitate groups utilizing principles consistent with:
    • Cognitive Behavioral Therapy (CBT)
    • Core Correctional Practices
    • Risk-Need-Responsivity principles
  • Evidence-based treatment models
  • Fill in as necessary for Residential Security Officer duties and provide transport for the client as needed.
Qualifications:
  • Experience working with individuals with:
    • Substance use disorders
    • Mental health conditions
    • Justice involvement
    • Behavioral challenges
  • Strong written and verbal communication skills.
  • Ability to work collaboratively within a multidisciplinary environment.
  • Ability to manage high volume client interactions in a group setting.
  • Proficiency in Microsoft Office.
  • Must be able to sit, stand and walk for extended periods of time
  • Occasionally lift/move up to 25 pounds.
  • Valid driver’s license and clean driving record
  • Must complete TB test, and drug screening (on-site).
  • Completion of CPR, First Aid, CBRF Fire Safety, and Choking training within 90 days
  • Employment is contingent upon passing a state and federal background check.
