Position Title: Social Worker

Location: Waconia, Minnesota

Facility Type: Long-Term Care Facility

Shift: Monday – Friday, 8:00 AM – 4:30 PM

Pay Range: $1900-$2070 (rate may adjust if housing is provided)

Start Date: 12/8/2025

Responsibilities

• Provide psychosocial support and counseling services to residents and families in a long-term care setting.
• Assess residents’ social, emotional, and psychological needs to develop individualized care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive resident care.
• Assist residents and families with care transitions, discharge planning, and community resource referrals.
• Advocate for residents’ rights and ensure their needs are met in accordance with facility policies and state regulations.
• Document assessments, interventions, and outcomes accurately in medical records.
• Provide crisis intervention and support during challenging situations.
• Facilitate communication between residents, families, and healthcare providers.
• Educate residents and families on available services, benefits, and support programs.
• Participate in quality improvement initiatives and compliance with regulatory standards.

Requirements

• Current Social Work license in the state of Minnesota.
• Bachelor’s degree in Social Work required.
• Minimum of 1 year of experience in a long-term care or skilled nursing facility preferred.
• Strong knowledge of state and federal regulations related to long-term care.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently and collaboratively with interdisciplinary teams.
• Experience with electronic medical records (EMR) systems preferred.
