Role: Speech Language Pathologist providing personalized care to older adults in private homes and senior living communities.
Responsibilities: Conduct evaluations, develop treatment plans, deliver evidence-based therapy to support patients' wellness, mobility, and independence. Document progress in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications: Valid SLP license or eligibility, degree from an accredited program, proficiency in computer and EMR systems, current CPR certification.
Skills & Physical Requirements: Ability to exert medium physical effort, assist with patient transfers, perform CPR, reposition patients, and drive to patient locations. Must meet physical demands of in-home care and wear PPE as needed.
