What are the responsibilities and job description for the Physical Therapist position at Rock Health?
Role: Licensed Physical Therapist providing in-home and senior living care to older adults, utilizing a flexible, patient-centered approach.
Responsibilities: Conduct evaluations, develop personalized treatment plans, deliver evidence-based therapy sessions to improve mobility, independence, and wellness. Document care accurately in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications: Valid state license or eligibility, degree from an accredited program, basic computer and EMR proficiency, current CPR certification.
Physical Demands: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, stand for extended periods, climb stairs, and drive between locations. PPE use required as needed.
