Physical Therapist (Sign-On Bonus)

Role Overview: Licensed Physical Therapists provide personalized, one-on-one care to older adults in private residences and senior living communities, focusing on improving mobility, independence, and quality of life.

Key Responsibilities

• Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions.
• Support patient wellness, functional independence, and aging in place through therapy and education.
• Document care accurately within electronic medical records, adhering to regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Provide education to patients, caregivers, and staff to enhance safety and function.
• Incorporate telehealth and virtual care to extend access and flexibility.
  
  

Qualifications & Skills

• Valid PT license in the practice state(s) or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Proficiency in basic computer use and EMR systems.
• Current CPR certification.
  
  

Physical & Other Requirements: Ability to perform moderate physical effort, assist with patient transfers, and meet the physical demands of in-home care, including lifting, standing, walking, and driving.
