Key Responsibilities
- Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions.
- Support patient wellness, functional independence, and aging in place through therapy and education.
- Document care accurately within electronic medical records, adhering to regulatory standards.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
- Provide education to patients, caregivers, and staff to enhance safety and function.
- Incorporate telehealth and virtual care to extend access and flexibility.
- Valid PT license in the practice state(s) or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Proficiency in basic computer use and EMR systems.
- Current CPR certification.
