We are seeking licensed Occupational Therapists to provide one-on-one care to older adults in senior living communities, private residences, and virtual settings.

- Conduct evaluations and develop personalized treatment plans
- Deliver evidence-based therapy to improve mobility, independence, and quality of life
- Document patient progress accurately in electronic medical records
- Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging in place strategies
- Utilize telehealth and virtual care to enhance access and flexibility
- Valid OT license or eligibility in practice state(s)
- Degree from an accredited OT program
- Proficiency in basic computer and EMR systems
- Current CPR certification
- Ability to exert moderate physical effort, including lifting and assisting patients
- Perform patient transfers, repositioning, and therapeutic activities
- Drive between patient locations and wear PPE as needed
