The provider conducts comprehensive in-home health assessments for adult and geriatric patients, reviewing medical history, medication adherence, social factors, and health risks to improve care quality and close care gaps.
Key Responsibilities Include
- Performing assessments, documenting findings, and providing health education according to protocols
- Administering and ordering point-of-care and lab tests as needed
- Collaborating with physicians, case managers, social workers, and caregivers
- Supporting patient follow-up and referrals, including physician communication
- Providing services at members' residences, including homes and Skilled Nursing Facilities
- Master’s Degree or equivalent experience with NP licensure and prescriptive authority
- Board certification in relevant adult or family care specialties
- Current BLS, ACLS, or CPR certification
- At least one year of NP experience preferred; new grads encouraged
- Valid driver’s license, automobile insurance, and strong computer skills
- Excellent communication skills
