Role: Home and Community Based Services Licensed Practical Nurse (LPN)
Responsibilities: Provide direct nursing care in patients' homes per physician’s orders, document patient visits thoroughly, coordinate care activities, and communicate effectively with patients, families, and healthcare team members. Work under supervision of a Registered Nurse or designated supervisor, ensuring high-quality clinical services within legal scope, adhering to infection control procedures, and participating in care coordination and case conferences.
Qualifications: Current, unrestricted LPN licensure, CPR certification, valid driver’s license or access to transportation, and ability to serve diverse home environments. For Louisiana, at least 1 year of LPN experience is required.
Preferred Skills: Home care experience, ability to work independently, manage multiple tasks, and strong communication, organizational, and writing skills. Flexibility to meet client needs is advantageous.
