Home and Community Based Services Licensed Practical Nurse
Role: Provide direct nursing care to acutely or chronically ill patients in their homes, documenting progress and coordinating care activities to ensure high-quality patient care. Work under supervision of a Registered Nurse or designated supervisor.

Responsibilities
- Deliver clinical services within the scope of practice, following the care plan and infection control protocols.
- Accurately document patient visits in electronic medical records and visit verification systems.
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, healthcare providers, and team members.
- Current, unrestricted LPN license and CPR certification.
- Valid driver’s license, vehicle insurance, or access to transportation.
- Ability to work in diverse home environments regardless of background.
- Minimum of one year of LPN experience, with state-specific licensure in LA.
- Home care experience, independent work skills, and strong communication and organizational abilities.
- Flexibility to meet client needs and manage multiple tasks.
