LPN Pediatric Home Care Night Shift

Home and Community Based Services Licensed Practical Nurse

Role: Provide direct nursing care to acutely or chronically ill patients in their homes, documenting progress and coordinating care activities to ensure high-quality patient care. Work under supervision of a Registered Nurse or designated supervisor.

Responsibilities

• Deliver clinical services within the scope of practice, following the care plan and infection control protocols.
• Accurately document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, healthcare providers, and team members.
  
  

Qualifications

• Current, unrestricted LPN license and CPR certification.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Ability to work in diverse home environments regardless of background.
• Minimum of one year of LPN experience, with state-specific licensure in LA.
  
  

Preferred Skills

• Home care experience, independent work skills, and strong communication and organizational abilities.
• Flexibility to meet client needs and manage multiple tasks.
  
  

Compensation is hourly, ranging from $22.90 to $34.35, with benefits including healthcare, incentives, recognition programs, stock purchase options, and 401k, subject to eligibility.
