Medical Records Clerk

We are looking for a detail-oriented Medical Records Clerk to join our team in Sunnyvale, California. In this Contract-to-Permanent position, you will play a vital role in maintaining and organizing patient records while ensuring compliance with federal and state regulations. Your work will support the efficiency of healthcare operations and contribute to delivering high-quality patient care.

Responsibilities:
• Manage and maintain electronic medical records, ensuring accuracy and timely updates.
• Organize, file, and retrieve patient information in both physical and electronic formats.
• Process medical record requests from patients, healthcare providers, and authorized entities.
• Collaborate with branch office staff and community relations teams to obtain physician orders promptly.
• Ensure proper handling of patient demographics and physician information for accurate billing and claims submission.
• Assist with audits and reviews by preparing patient records and supporting documentation.
• Coordinate offsite storage and destruction of records, including purging outdated files.
• Maintain confidentiality by securely handling shredding and record destruction processes.
• Support quality improvement initiatives and participate actively in team activities.
• Perform other related duties as assigned to meet department and organizational goals.
• Proven experience in managing medical records and working with electronic health record (EHR) systems.
• Proficiency in using office software and technologies such as Allscripts or Cerner.
• Familiarity with medical terminology and healthcare documentation standards.
• Typing speed of 45-60 words per minute with high accuracy.
• Strong organizational skills and attention to detail.
• Ability to handle sensitive patient information with confidentiality.
• Knowledge of federal and state healthcare regulations related to medical records.
• Effective communication and teamwork skills.
