Job Overview:
We are seeking a dedicated Community Case Manager to join our team.
These responsibilities include, but are not limited to:
-Building rapport with clients, other staff, and community partners.
-Assisting clients with obtaining or renewing social service and income benefits.
-Documenting and managing program activities to meet organizational goals
-Performing assessments and periodic reviews to assess needs of clients and make recommendations for improvements
- Traveling to company owned and regulated locations
-Collaborate with internal teams to ensure program alignment with organizational objectives
- Plan and coordinate community engagement
-Maintaining a caseload of adult clients
- Advocating for clients with community providers and services
-Accompanying clients to appointments and ensuring they have the correct services in place (you are not required to use your own vehicle)
Qualifications:
- Bachelor's degree in Social Work, Behavioral Health, or related field
- 1 year of experience in mental health field
- QMHP or eligible
- Strong understanding of serving vulnerable populations
- Excellent organizational and time management skills
- Ability to multitask and prioritize tasks effectively
- Strong interpersonal skills for effective relationship management
-Drivers License
Schedule: Monday-Friday. (8am to 4pm or 9am to 5pm)
Location: Virginia Beach, in-office, but field Locations are expected to be attended as well (company vehicles available)
