Nurse Leader/Supervisor - LPN or RN M-F Evenings
At RiverWoods Durham, we are currently hiring a LPN or RN to join our team as a Nurse Leader. This role requires participation in the on-call rotation, which may include coverage of other shifts at times.
The ideal candidate would be available to work 4pm to 12:30am shift, Monday through Friday.
Key Responsibilities:
- To manage and coordinate the care of residents utilizing the nursing process.
- To collaborate with team members to formulate individualized resident care plans and update them as necessary.
- To assess and document changes in residents' physical or mental status and report findings to the primary care physician and treatment team.
- To provide leadership and direction for direct care partners and supervise direct care team members through observation and evaluation of direct care results and performance.
- To establish positive working relationships with team members, residents, and family members to facilitate open, direct communication.
Qualifications:
- Must possess, as a minimum, a Nursing Degree or Diploma from an accredited college, university, or nursing school.
- Long term care facility and specialized knowledge of nursing care of the elderly and rehabilitative nursing is preferred.
- Must hold a nursing license within the State of New Hampshire.
- The Nurse Leader must be Resident Assessment Tool certified.
