What You’ll Be Doing
- Conducting psychosocial assessments in patients’ homes
- Developing and implementing care plans
- Supporting patients and families with emotional and practical needs
- Educating on advance directives and care planning
- Coordinating with community resources and services
- Participating in interdisciplinary team meetings
- Providing support across hospice (with occasional home health collaboration)

What Helps You Succeed
- Master’s degree in Social Work (MSW)
- Licensed Clinical Social Worker (LCSW) in Oregon
- 2 years healthcare experience preferred (hospice or home health a plus)
- Current BLS certification
- Strong communication and organizational skills
- Valid driver’s license and reliable transportation
This role includes travel throughout Jackson County and surrounding areas, providing care in homes and facilities.
