The Opportunity- Men's Recovery Home
Tuesday - Saturday 3-11pm
The Case manager is responsible for all intake, assessment, treatment planning, and progress note documentation for each client assigned to their caseload. Case managers work collaboratively with the Clinical Director and Program Director to provide the most clinically appropriate addiction and mental health services to match client level of motivation and preference for ongoing recovery.
Responsibilities
1. Supervise and monitor the health and safety of clients in the program.
2. Responsible for maintaining the staffing ratio as assigned by the Program Manager; (remaining on-site as directed by supervisor).
3. Attending and participating in weekly clinical and staff meetings.
4. Complete new client intake assessments and additional intake paperwork as assigned.
5. Collaboratively develop client treatment plans for an assigned caseload and oversee implementation through utilization of motivational techniques.
6. Develop and implement aftercare plans in collaboration with team members.
7. Responsible for monitoring assigned caseload participation in activities, groups, program structure, etc. through documentation review and communication with team members.
8. Monitor assigned caseload for changes that warrant clinical and medical interventions.
9. Participation in client treatment meetings.
10. Facilitate and monitor progress through motivational phases
11. Facilitate clinical treatment groups as assigned utilizing approved curriculum and other best practice standards.
12. Provide crisis intervention and de-escalation for clients in distress, including overdose prevention.
13. Oversee electronic health record for each client on the assigned caseload, including assisting with quality assurance reviews and audits.
14. Participate in your performance evaluation with the Clinical Director.
15. Assist in oversight of Recovery Specialist, direct care, or interns when on shift, in the absence of program supervisors.
16. Assist in day-to-day operations of the program as needed
17. Have knowledge and understanding of DPH regulations and Riverbend Policy and Procedure manuals.
18. Responsible for the immediate notification of safety/health/legal issues happening in the program (client or programmatic), including completing an Internal or Critical Incident Report form.
19. Responsible for timely calling out to identified supervisor due to inability to cover the shift.
20. Responsible for completing and authorizing weekly timesheets with your signature to accurately reflect hours worked on shift.
21. Other duties as assigned.
Preferred Education and experience
Bachelor’s Degree in Human Services, Addictions, Psychology, or related field, preferred. High school diploma minimum requirement.
