Primary Duties:
- Provides support to the Administration to ensure residents and family have appropriate needs met with social setting.
- Assist resident with care planning, schedules, concerns, notifications.
- Ensure all staff and visitors check in and out of the kiosk system.
- Exhibits polite and professional communication via phone, e-mail, and mail.
- Assist Admissions coordinator as needed with referrals
- Provide tours as needed.
- Assist in other duties as needed.
License/Certification:
- Minimum of a Bachelor’s degree (BSW/MSW) in social work; OR
- Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
- One year in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals
