Our LTC facility seeks a bright, kind, professional individual to serve as the Assistant MDS Coordinator. You will assist our MDS Coordinator and the care team in this role.
Job Summary
- Assist the MDS Coordinator in completing and updating MDS forms on all patients in the care facility.
- Help the MDS Coordinator care for each resident’s unique needs.
- Per instructions from the MDS Coordinator, perform clinical assessments to gather information on residents.
- Assist in developing care plans for patients to coordinate available services.
- Submit updates and paperwork to maintain compliance
- Coordinator and attend meetings with other care team families
- Assist the MDS Coordinator in educating patients and their families
- Other Assigned duties
