What are the responsibilities and job description for the Social Worker position at River Hills Health and Rehab?
River Hills Health and Rehab is seeking a compassionate and knowledgeable Social Worker to support the psychosocial well-being of our residents in a skilled nursing and long-term care setting. This position plays a key role in ensuring compliance with Texas Health and Human Services (HHSC) and federal regulations while promoting person-centered care.
Responsibilities:
- Complete comprehensive psychosocial assessments upon admission and as needed
- Develop, implement, and update individualized care plans in coordination with the interdisciplinary team
- Provide emotional support and counseling to residents and families
- Assist with discharge planning, including coordination with community resources and services
- Act as a liaison between residents, families, and facility staff to ensure effective communication
- Manage and document resident grievances in accordance with facility policy and regulatory requirements
- Assist residents with advance directives and ensure proper documentation is maintained
- Support PASRR-related processes and ensure appropriate documentation and coordination
- Participate in care plan meetings, QAPI activities, and survey readiness efforts
- Maintain accurate, timely, and compliant documentation in the medical record
Qualifications:
- Bachelor’s degree in Social Work or related field required (BSW preferred; MSW a plus)
- Must meet Texas requirements for a nursing facility social worker
- Previous experience in long-term care or healthcare setting preferred
- Knowledge of Texas HHSC regulations and federal nursing facility guidelines
- Experience with PASRR, discharge planning, and advance directives preferred
- Strong communication, organizational, and problem-solving skills
- Ability to handle sensitive situations with professionalism and compassion
- Must be able to pass background screening and meet health requirements
