The Admissions Care Coordinator will work closely with Admissions Clinicians to support new clients as they enter services. This role ensures that individuals’ behavioral health, physical health, and other identified needs are addressed through effective coordination with River Edge programs, primary care, and community partners. The Care Coordinator plays a vital role in supporting smooth transitions, maintaining communication across systems, and promoting recovery-focused, person-centered care.

Key Responsibilities
- Provide care coordination for all newly admitted clients in collaboration with Admissions Clinicians.
- Collect demographic information and accurately enter data into the Electronic Medical Record.
- Participate in assessments and assist with the development of person-centered treatment plans.
- Engage in follow-up activities with River Edge programs, community partners, and primary care providers to ensure quality care.
- Communicate effectively with River Edge staff and external providers to resolve issues and facilitate smooth service transition
- Bachelor’s degree in social work, psychology, criminal justice, or another healthcare-related field.
- Knowledge of mental illness, intellectual/developmental disabilities, and substance use disorders.
- Understanding of co-occurring medical conditions and recovery-focused care.
- Strong interpersonal skills with the ability to work collaboratively.
- Flexibility to assist other departments as needed.
- Familiarity with mental health and substance use services and local community resources.
- Customer service experience or prior experience in a behavioral health setting.
