Responsibilities:
Provide social work services to patients and their families, including counseling, discharge planning, and connecting them with community resources
Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of each patient
Conduct social and psychological assessments to identify patients' social and emotional needs
Develop and maintain relationships with community resources, organizations, and agencies to ensure patients receive necessary support
Document patient interactions, assessments, and progress notes in accordance with established policies and procedures
Participate in quality improvement initiatives to ensure high-quality care and patient satisfaction
Requirements:
Bachelor's degree in Social Work or related field (or Master's degree preferred)
2 years or more of experience in social work, healthcare, or a related field
Licensure as a Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the State of Texas (or ability to obtain within 6 months of hire)
Strong communication, interpersonal, and organizational skills
Ability to work effectively in a fast-paced environment with multiple priorities
