Case Manager / Discharge Planner

Full-Time (Hours and Days may Vary as Needed)

As a Member Of The Multi-disciplinary Treatment Team, The Discharge Planner Is Responsible For All Aspects Of The Dischagre Planning Process Including:

I. Discharge Planning

  • Coordinates discharge planning by initiating discharge plan upon admission, reviews estimated length of stay, identifies discharge problems with treatment team during treatment team meetings and maintains contact with program therapist and physician to assist in aftercare planning.
  • As necessary directly assists patients in obtaining resources, placement options and aftercare arrangement. Aides in completion of referral packets and other requirements to assure placement or aftercare are completed.
  • Schedules aftercare appointment for assigned patient caseload prior to the patient's discharge based on the clinical needs and financial parameters of the family and/or guardian.

II. Communication

  • Provides written documentation as needed to referral sources, courts, aftercare providers, shelters, group homes, etc., in order to transition patients to other levels of care as indicated by the treatment team.
  • Provides clinical updates to outside entities in written form as necessary during the patient's hospitalization, as well as aftercare recommendations at the time of discharge.
  • Communicates with the case managers and other departments regarding placement problems and payor issues that impact discharges.
  • Maintains a good working relationship with referral sources as well as knowledge of community resources.

III. Treatment Team Participation

  • Attends and participates in treatment team staffing(s) and continuity of care.
  • Actively alerts treatment team of any issues related to discharge planning.

The DischargePlanner interfaces with referral sources, community service agencies, businesses, employers, and healthcare facilities; ensures compliance for quality patient care and compliance with the Joint Commission standards, State Health Department requirements, and all other regulatory standards.

Qualifications

  • Master’s Degree in Social Work, Counseling, or Counseling Psychology from an accredited university.
  • Appropriate State Licensure including LMSW, LCSW, LAC, LPC, LPE. Licensure must be maintained.
  • A minimum of two years psychiatric treatment experience is preferred along with experience in individual, group, and family therapy.
  • Knowledge of regulatory requirements and Arkansas laws governing services to adults preferred.
  • A basic knowledge of educational, psychological and social service systems as related to special education and behavior treatment preferred.
  • Current CPR or BLS certification. (offered at facility).
  • Current Handle with Care training and Verbal De-escalation training. (offered at facility).
