What are the responsibilities and job description for the Resource Specialist position at Rising Ground, Inc.?
Job Summary: The Resource Specialist will support children and family members in various ways, including conducting home visits, conducting school visits, and accompanying parents to service providers. The Resource Specialist will assist therapists in identifying resources that are supportive to families and making referrals to needed services. Additionally, the Resource Specialist will also support the program with administrative duties by reviewing case records, archiving closed cases etc.
Job Responsibilities:
- Participate in mandatory once a week individual and unit supervision; attend all mandatory training courses, general staff meetings, as well as serve on committees as assigned.
- Attend Outreach meetings and events as needed
- Do case record reviews and report out missing documentations to supervisors
- Create partnerships with outside service providers and make referrals as needed
- Conduct in-home, school, and virtual visits for therapists as needed and complete documentation
- Attain school and medical reports; document in Connections utilizing appropriate progress note template and file in case record. Place printed copies of all important documents in case files.
- Archive case records and other general administrative duties as needed
- Maintain consistent communication with therapists and supervisors around information needed for records, referrals, and visits
- Perform other duties as assigned
Qualifications:
- High School Diploma or Associates plus a minimum of two years of prior experience in child welfare.
- Must demonstrate strong organizational and computer skills including proficiency in Microsoft Word Outlook, Excel PROMIS and Connections.
- Driver’s license a plus.
- Ability to be flexible with schedule working late night once a week as needed
- Bi-lingual (English and Spanish) Preferred
Supervisory Responsibility: No
Work Environment: Queens Office, with one (1) remote day every two (2) weeks.
Position Type/Expected Hours of Work: Full time position, scheduled appropriate to the needs of client and families. This position calls for a thirty-five (35) hour work week, but more than 35 hours per week may from time to time be required.
Travel (if required): Although the main location is the Queens office, travel required to Rising Ground sites in the Bronx, Harlem and Queens, including home visits. Work-related travel expenses will be reimbursed by an agency.
Physical Requirements: Must have the ability to complete all standard administrative and support tasks including but not limited to climbing up and down stairs, operation of computers/phones/fax/printer/copy machine, and the ability to lift boxes, furniture, and equipment up to 50lbs. Must be able to travel from the various locations and sites of Rising Ground.
