Role Overview
The LPN is responsible for supervising day-to-day health services activities. Health Services provide services and support for foster care, residential and ACS involved youth..
Key Responsibilities
- Provides general medical care and treatment to individual supported per program, on site.
- Performs physical examinations and preventative health measures within prescribed guidelines and instructions.
- Provide health evaluations, assessments, and screenings with follow-ups.
- Review all Pediatric guidelines for care, including immunizations and testing
- Records physical findings based on the patient’s condition.
- Participate in clinical team and other health professionals to prepare comprehensive patient care plan utilizing historical and current information
- Conducts and assists in medical examinations of youth after incidents.
- Provides health education to youth supported, including reproductive health education
- Administer Medication
- Train staff
- Assist with community provider visits as needed
- Other related duties as assigned by supervisor.
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in New York State, in good standing.
- Minimum of 2 years of nursing experience in a clinical, community-based, or related healthcare setting.
- Current Basic Life Support (BLS) certification required.
- Demonstrated ability to perform comprehensive nursing assessments and exercise sound clinical judgment in an autonomous work environment.
- Strong critical thinking and problem-solving skills, with the ability to evaluate and interpret medical records, laboratory reports, and other clinical documentation.
- Experience triaging patient calls, assessing urgency, and coordinating appropriate follow-up care.
- Ability to provide guidance and support staff as needed while maintaining high standards of patient care.
