What are the responsibilities and job description for the Resource Coordinator position at RISE Community Solutions?

PURPOSE: The Resource Coordinator strengthens Breakthrough’s presence and service delivery across Osceola and Orange counties by building and maintaining strong partnerships with community agencies, schools, hospitals, and behavioral health providers. The Resource Coordinator ensures timely, coordinated access to services for youth and families through outreach, referral network development, and warm handoffs between agencies.
KEY RESPONSIBILITIES:
- Build and maintain partnerships with schools, hospitals, mental health providers, and community organizations.
- Conduct consistent outreach to expand Breakthrough’s referral network.
- Identify eligible youth for program services and initiate warm handoffs between agencies.
- Represent Breakthrough at community events, meetings, and collaborative initiatives.
- Assist families in navigating mental health, social service, and educational systems.
- Coordinate timely referrals and ensure smooth transitions between service providers.
- Maintain up-to-date knowledge of local resources in Osceola and Orange counties.
- Support families in minimizing barriers to care, including language access and service eligibility.
- Provide services in both English and Spanish.
- Build trust with families through culturally informed practices.
- Maintain accurate records of outreach, referrals, and partnerships.
- Contribute to program enhancement through feedback and strategic recommendations.
- Support data collection and reporting requirements as needed.
· Complete timely and accurate documentation, including case notes and referral forms.

- Identify opportunities in which the lived experience and voice of those served can improve program operations and outcomes.
· Participate in the organization’s strategic plan and quality improvement processes.
· Other duties as assigned in support of the mission of the organization.
DESIRED QUALIFICATIONS:
REQUIRED:
· High school diploma or GED.
· Fully fluent in Spanish and English (verbal and written). Because a significant percentage of families served identify Spanish as their primary language, this position requires full fluency in Spanish to ensure equitable access, effective communication, and culturally responsive support.
· Knowledge of mental health, social services, and local systems of care in Central Florida.
PREFERRED:
· Experience working with diverse and underserved communities.
· Familiarity with trauma-informed and recovery-oriented practices.
· Experience in care coordination, case management, or community outreach.
· Parent or caregiver of a child or adolescent with a serious mental health need, who has previously received or is currently receiving services from the mental health service system.
· Registered with the Florida Certification Board as a Certified Recovery Peer Specialist or meets the qualifications to be certified. Click here to learn more about the CRPS requirements. Obtaining a CRPS is not a requirement of this position, but the agency will support costs and preparatory support should the Resource Coordinator pursue the certification.
DESIRED SKILLS:
- Excellent interpersonal communication and writing skills; must be able to work well with community stakeholders, participant families, senior managers and providers.
- Must be well organized and capable of completing assignments with limited guidance and supervision.
- Proficient in the use of MS Word and Outlook, and virtual meeting platforms (i.e., MS Teams).
- Ability to learn and utilize database reporting systems.
- Demonstrate cultural and linguistic competence and sensitivity to population served.
- Ability to travel in personal vehicle (mileage reimbursement applies).

POSITION STATUS: Non-Exempt
SCHEDULE: Traditional weekday schedule with some evenings required based on needs of assigned families. In such circumstances, a practice of flex scheduling will be implemented to ensure this role is not working more than the required full-time hours.
LOCATION: This role can be worked in a hybrid capacity but requires regular local travel. The employee will be able to use their home or the RISE Community Solutions office in Kissimmee, FL, for “desk” tasks. Team members meet approximately two days per month in-person at the RISE Community Solutions main office in Maitland, FL.

Pay: $44,000.00 - $55,000.00 per year
