Job Overview

Rise Above Recovery is seeking a compassionate, organized, and client-centered Case Manager to join our growing behavioral health team. As a Case Manager, you will work closely with clients, behavioral health providers, medical professionals, justice agencies, community partners, and family members to ensure clients receive coordinated, person-centered care. Duties

  • Conduct comprehensive client assessments to identify strengths, needs, barriers, and goals related to behavioral health, recovery, housing, employment, education, transportation, and social support.
  • Develop individualized service plans and assist clients in accessing appropriate community resources and support services.
  • Coordinate care with multidisciplinary teams, including counselors, medical providers, probation officers, social service agencies, and family members.
  • Assist clients in navigating healthcare systems, public assistance programs, housing resources, and community-based services.
  • Provide advocacy and support to clients as they work toward recovery, stability, self-sufficiency, and personal goals.
  • Monitor client progress and update service plans as needed to reflect changing circumstances and treatment goals.
  • Facilitate referrals and ensure continuity of care throughout treatment, transitions, and discharge planning.
  • Maintain timely and accurate documentation in accordance with agency policies, HIPAA regulations, and applicable state standards.
  • Support clients involved in the justice system by assisting with compliance requirements, service engagement, and successful community reintegration.
  • Participate in team meetings, case consultations, and ongoing professional development opportunities.

Qualifications

  • Minimum of two (2) years of experience working in behavioral health, substance use treatment, social services, human services, or a related field.
  • Current CDC I certification preferred; candidates who do not yet hold a CDC I must be eligible for and committed to obtaining CDC I certification within two (2) years of hire.
  • Knowledge of case management principles, recovery-oriented systems of care, and community resources.
  • Experience working with individuals experiencing substance use disorders, mental health conditions, homelessness, justice involvement, or other complex life challenges preferred.
  • Strong organizational skills with the ability to manage multiple cases and priorities effectively.
  • Excellent communication, advocacy, and interpersonal skills.
  • Ability to work effectively with diverse populations while maintaining confidentiality and professional boundaries.
  • Proficiency with electronic health records (EHR/EMR), Microsoft Office applications, and documentation requirements.
  • Valid driver's license and ability to travel locally, as needed.
